What Is Liposuction?

Liposuction is a surgical body contouring procedure that removes stubborn fat deposits unresponsive to diet and exercise. It is most commonly performed on the thighs, hips, abdomen, and knees in women, and the abdomen and waist in men. Other areas such as the neck, arms, back, calves, and ankles can also be treated. While sometimes referred to as liposculpture, micro-liposuction, or lipoplasty, all of these terms refer to the same technique. Liposuction is not a weight-loss solution but a method for refining body shape.

Is Liposuction Safe?

Yes, liposuction is safe when performed by a properly trained, board-certified plastic surgeon on appropriate candidates. Complications typically arise in rare cases involving poor patient selection, excessive fat removal, or combining too many procedures at once.

Who Is a Good Candidate?

Ideal candidates are individuals with:

  • A stable, healthy weight
  • Good skin elasticity
  • Localized fat deposits
  • Realistic expectations

While younger patients tend to achieve the best results due to skin tone, patients over 40 can also benefit if their skin remains firm. Those who are significantly overweight may see more modest outcomes.

Understanding BMI and Surgical Safety

Body Mass Index (BMI) helps determine if a patient’s weight is appropriate for surgery. A BMI over 30 is considered obese and increases the risk of complications such as poor healing or adverse reactions to anesthesia. Patients with a high BMI are advised to lose weight before surgery.

Liposuction for Men

In men, liposuction is most often used to contour the abdomen and waist. Candidates should be close to their ideal weight. Men with internal abdominal fat will not benefit from liposuction. This procedure is also used as part of gynecomastia (male breast reduction) surgery and can enhance neck contours when combined with muscle tightening.

How Is Liposuction Performed?

Before surgery, target areas are carefully marked with concentric circles indicating the zones of fat accumulation. Small incisions (about ¼ inch) are made in inconspicuous locations. A tumescent solution—containing anesthetic and epinephrine—is injected to reduce bleeding and discomfort.

Fat is then removed using hollow tubes called cannulas, connected to a vacuum. Cannulas vary in size depending on the area and degree of precision required. The surgeon passes the cannulas in multiple directions to ensure even fat removal and a smooth contour.

Types of Liposuction Techniques

  • Tumescent Liposuction (Gold Standard): Traditional, effective, and safe.
  • Power-Assisted Liposuction (PAL): Uses a motorized cannula to reduce surgeon fatigue and may cause less bruising.
  • Ultrasound-Assisted Liposuction (UAL): Uses sound waves to break up dense fat, useful for areas like male flanks or previously treated regions.
  • Laser-Assisted Liposuction (e.g., SmartLipo): Adds laser energy to emulsify fat. Though heavily marketed, studies show no clear advantage over traditional methods and risks include skin burns.

What About Nonsurgical Alternatives?

  • CoolSculpting: Freezes fat cells for gradual elimination. Results are modest and multiple treatments are required.
  • Mesotherapy/Lipodissolve: Involves injecting chemicals to dissolve fat. These substances are not FDA-approved, and adverse effects like scarring and skin ulceration have been reported.

While nonsurgical methods avoid anesthesia, they typically offer less dramatic and less predictable results than liposuction.

Fat Grafting for Breasts and Buttocks

Liposuction fat can be repurposed for fat transfer to the breasts or buttocks. While this can improve shape and volume without implants, the results are more subtle. Not all transferred fat survives, and repeat sessions may be necessary. In the buttocks, fat grafting is preferred over implants but must be performed with great care to avoid serious complications such as fat embolism.

How Long Do Results Last?

Liposuction results are permanent because the number of fat cells is reduced. However, significant weight gain can still increase fat in other areas. Maintaining a healthy lifestyle is essential to preserving results.

Recovery and Aftercare

  • Activity: Light activity after 1-2 days; return to work in about 1 week; avoid vigorous exercise for 3 weeks
  • Massage: Lymphatic drainage massage is optional and not required

Risks and Complications

Complications are rare but can include:

  • Infection or bleeding
  • Fat embolism
  • Contour irregularities or asymmetry
  • Numbness or patchy sensation (often temporary)

Smoking greatly increases the risk of healing problems. All nicotine use should be stopped 3 weeks before and 6 weeks after surgery.

What Are the Limitations of Liposuction?

  • Not a substitute for weight loss
  • Doesn’t improve cellulite or loose skin
  • Results depend on body type, skin tone, and age
  • Best outcomes in patients with tight skin and localized fat

Some patients may require touch-up procedures for optimal symmetry and contour. Liposuction is a powerful tool but works best when part of a healthy lifestyle.
